    Penile fracture (PF) as a rare condition is one of the most important urological injuries that occurs following blunt trauma to the erect penis. It usually presented with unilateral corpus cavernosa injury without urethral involvement.We report a 24-year-old man who presented by PF with bilateral corpus cavernosa tears and complete disruption of the urethra and corpus spongiosum following anal intercourse. Bilateral corpus tears and urethral disruption was repaired surgically within 3 hours of the trauma. In 12 months follow up, the patient has a normal sexual activity and voiding. Penile fracture commonly involves unilateral corpus cavernosa without urethral involvement. We reports a case of massive injury to bilateral corpus cavernosum and corpus spongiosum with complete rupture of the urethra following anal intercourse which is extremely rare. As the first step of management of PF, surgical approaches without wasting time could have a great outcome.
